Administration |
Many of the plans that make up Valassis' benefits program are subject
to the requirements of the Employee Retirement Income Security Act of
1974 (commonly called ERISA). Each of the following plans is classified
as a welfare plan:
- Medical
- Dental
- Vision
- Short-term Disability Insurance
- Long-term Disability Insurance
- Long-term Care Insurance
- Associate Basic Life Insurance
- Dependent Term Life Insurance
- Accidental Death & Dismemberment (AD&D) Insurance
- Flexible Spending Accounts (FSAs)—before-tax contributions
for medical and dependent care expenses
- Employee Assistance Program (EAP)
This section provides information about the administration of these
ERISA benefit plans. It also explains certain rights that you have under
the plans, for example, what actions you may take if a claim is denied.

Plan Trustee
The Valassis Retirement Savings Plans with JP Morgan are administered
by separate written plans and separate trust agreements, and the trustees
of those agreements are responsible for the management of the plan's
assets.

The plan year for the various plans is January 1 through December 31.

Each plan has an official name, and a description of each is on file

Insured Plans
The welfare benefit plans listed below are a combination of self-insured
plans and plans insured through contracts with insurance companies.
Contributions for coverage may be made solely by Valassis or by participating
associates. Some of the plans require joint contributions from participating
associates and Valassis. The insurance companies listed below provide
the coverages and process claims.
