FLEXIBLE SPENDING ACCOUNTS
Filing a Claim

To receive reimbursement from your Health Care or Dependent Care FSA, submit a claim form along with written evidence of your incurred expense to the WageWorks Claims Office, at the address shown on the form. Reimbursements will be made up to two times per month. You will receive your payment as soon as administratively possible, along with an explanation of benefits.

If you have a medical expense not covered by the plans, you will need to file a claim for reimbursement using the Health Care FSA Reimbursement claim form.

You may use the money deposited in your FSA account for expenses incurred during the plan year, from January 1 through March 15th of the following year. Any expenses you may have prior to participation in the plan are not eligible for reimbursement.

Reimbursement will be made from each account as follows:

For the Health Care FSA

You can receive reimbursement from your Health Care FSA, up to the total amount of your annual contributions, regardless of the amount in your account at the time you request payment. For example, if you are contributing $1,200 annually to the account, have an account balance of $200 in February, and incur eligible medical expenses of $600 in February you may be reimbursed for the entire $600. You will continue to make monthly contributions to your account throughout the rest of the year.

For the Dependent Care FSA
You can receive reimbursement from your Dependent Care FSA, up to the amount in your account at the time you request payment. For example, if you elect to contribute $1,200 to your Dependent Care account, have an account balance in February of $400 ($200 from you and $200 in matching company contribution), and submit a claim for $500 worth of eligible dependent care expenses, you will receive a $400 reimbursement at that time. For Both FSAs
You have until April 30th of the following year to submit claims for reimbursement for expenses that you incurred during the plan year and through March 15th of the next plan year. For example, if you participated in the Health Care FSA in 2008, you have until April 30, 2009, to submit claims you incurred from January 1, 2008 through March 15, 2009.

Direct Deposit of Reimbursements
You may elect to have your FSA reimbursements deposited directly to your checking or savings account. If you elect this feature, you will not have to wait for your reimbursement checks, and funds deposited to your account will be available immediately.
