LIFE INSURANCE
Dependent Term Life Insurance

Part- and full-time associates who are scheduled to work 20 hours a week or more may purchase insurance separately for your spouse, civil union or domestic partner and children through the Valassis Dependent Term Life Insurance program. You pay for this coverage through payroll deductions.

You can choose between coverage of $50,000 for your spouse, civil union or qualified domestic partner and/or $10,000 for each eligible child $30,000 for your spouse, civil union or qualified domestic partner and/or $10,000 for each eligible child or $10,000 for your spouse, civil union or domestic partner and/or $5,000 for each eligible child.

If your spouse, civil union or qualified domestic partner or child should die from any cause, the dependent life insurance benefit will automatically be paid to you.

Evidence of Insurability
If you do not elect dependent life insurance within 31 days of your date of hire, you must provide evidence of insurability to enroll your spouse for this coverage. Once you enroll, new dependent children (through marriage or the birth of a child, for example) are automatically covered.

All associates should complete the Valassis Personal Health Application. This form can be submitted to Sun Life for review and approval at the address on the form.

Deferred Effective Date:
When will the effective date for Dependent coverage or a change in coverage be deferred?

If, on the date Your Dependent, is to become covered:

  • under the Policy;
  • for increased benefits; or
  • for a new benefit;

he or she is:

  • confined in a hospital; or
  • confined elsewhere;

such coverage will not start until he or she:

  • is discharged from the hospital; or
  • is no longer confined elsewhere;

and has engaged in all the normal and customary activities of a person of like age and gender, in good health, for at least 15 consecutive days.

This Deferred Effective Date provision will not apply to disabled children who qualify under the definition of Dependent Children.

Confined Elsewhere means your dependent is unable to perform, unaided, the normal functions of daily living, or leave home or other place of residence without assistance.

Qualified Change in Status
If you have a qualified change in status, you may be able to enroll your spouse, civil union or domestic partner within 31 days of the event, without providing evidence of insurability if they are newly eligible. If you are already covered, you may increase your coverage one level without providing evidence of insurability. You may also be able to decrease or cancel coverage. Qualified changes in status generally include changes in:

  • Your marital status, including marriage, civil union, divorce, legal separation, annulment or death of a spouse (no evidence of insurability required);
  • The number of your dependents, including birth, adoption, placement for adoption or death or a dependent;
  • Employment status for you, your spouse or dependent, such as starting a new job or stopping employment (evidence is required for your spouse, civil union or domestic if you do not currently have coverage;
  • Work schedule, such as a reduction or increase in hours, a switch between part-time and full-time employment or the start of, or return from, unpaid leave; (evidence is required for your spouse, civil union or domestic if you do not have coverage; or
  • Dependent status (for example, your dependent child over age 19 is no longer a full-time student)

If Valassis Communications, Inc. or NCH associates have a qualified change in status, they must contact their HR Representative. Valassis Direct Mail and Sales and Marketing associates must make the appropriate change on the enrollment site or notify the Associate Resource Center immediately to report the change. Any forms must be completed and received by the Human Resources Representative or Associate Resource Center within 31 calendar days of the qualified change in status.
