VISION
How the Plan Works

The amount of your benefit differs when you receive your vision care services from an EyeMed provider or from a non-EyeMed provider. When you receive your vision care services from an EyeMed provider, you will receive the in-network benefit level. If you do not use an EyeMed provider, you will receive the lower, out-of-network benefit level, which is paid in the form of an allowance.

When you need eyecare services, here are the steps to take:

  • Call your EyeMed doctor and make an appointment. To locate a participating doctor in your area, log on to www.enrollwitheyemed.com/access or call 1-866-723-0596.

  • Tell the EyeMed doctor that you are a Valassis associate and provide your EyeMed Identification Card.

    The participating doctor will contact EyeMed to verify your eligibility and benefit coverage and obtain authorization for the necessary services and materials.

  • Go for your eye exam. Your EyeMed provider will determine if glasses or contact lenses are necessary. If you need glasses or contact lenses, ask your provider whether the services and materials are approved by EyeMed. If you elect more expensive materials for cosmetic reasons, you must pay the additional cost yourself.

  • Pay your doctor. When you receive services at a participating EyeMed provider location, you will not have to file a claim form. At the time services are rendered, you will have to pay the cost of any services or eyewear that exceeds any allowances, and any applicable co-payments. EyeMed pays the doctor directly for services and materials covered by your EyeMed plan. You will owe state tax, if applicable, and the cost of non-covered expenses (for example, vision perception training).

Using Non-EyeMed Providers
If you prefer, you may also receive vision care services from any licensed provider (optometrist, ophthalmologist or optician) who is not in the EyeMed network. Keep in mind, however, that when you receive services from a non-EyeMed provider:

  • Your benefits are limited
  • Your out-of-pocket costs may be higher
  • You will need to file a claim
