WHAT IF I...?
You or a Family Member Dies

In the event of your death while an Valassis associate, a family member should:

  • Notify Valassis. As soon as possible, a family member should notify your supervisor or manager, who will forward the information the Human Resources Representative Valassis Communications, Inc. and NCH associates paid through ADP) or to the Associate Resource Center (Valassis Direct Mail and Sales & Marketing associates paid through PeopleSoft).

  • File for insurance benefits. Your surviving spouse or dependents must file a claim to receive your:

    • Basic or Voluntary Life Insurance benefits

    • AD&D Insurance benefits, if you die as the result of an accident

     

    Your family will need to send a certified death certificate to your Human Resources Representative (Valassis Communications, Inc. and NCH associates paid through ADP) or the Associate Resource Center (Valassis Direct Mail and Sales & Marketing associates paid through PeopleSoft) or in order to process a life insurance claim in the event of your death. AD&D Insurance may apply as well if you die as a result of an accident.

  • Consider seeking support through the Employee Assistance Program. Contact Ceridian LifeWorks at 1-888-267-8126, user id: Valassis - password: Lifeworks. For a Spanish- speaking representative, contact 1-888-732-9020.
  • Obtain Retirement Savings Plan distribution forms or request a transfer of the Retirement Savings Plan account to the beneficiary on file. Contact JP Morgan at 1-800-345-2345.
  • Review Employee Stock Purchase Plan. Your Employee Stock Purchase Plan account will become part of your estate.

  • Consider electing COBRA coverage. Your spouse and/or dependents may be eligible to continue their medical, dental, vision and prescription benefits and the Health Care Spending Account under the provisions of COBRA.

If you suffer the loss of a spouse or dependent, you should:

  • Notify Valassis. As soon as possible, you should notify your Human Resources Representative (Valassis Communications, Inc. and NCH associates paid through ADP) or the Associate Resource Center (Valassis Direct Mail and Sales & Marketing associates paid through PeopleSoft).

  • Bereavement Leave. You may take up to three consecutive days of bereavement leave from work for the death of most family members.

  • File for insurance benefits. You must file a claim to receive:

    • Dependent Term Life Insurance benefits (if applicable). Contact your Human Resources Representative (Valassis Communications, Inc. and NCH associates paid through ADP) or the Associate Resource Center (Valassis Direct Mail and Sales & Marketing associates paid through PeopleSoft).

  • Review your benefit selections. You may need to change your benefit elections for yourself and your surviving spouse and/or dependents for:

    • Medical, dental and vision benefits

    • Dependent and Voluntary Term Life Insurance

  • Review FSA contributions. You can begin, stop or change the amount of your contributions. Contact your Human Resources Representative (Valassis Communications, Inc. and NCH associates paid through ADP) or the Associate Resource Center Valassis Direct Mail and Sales & Marketing associates paid through PeopleSoft).
  • Review your designated beneficiaries. You may change your beneficiaries for:

    • Basic and Voluntary Life and AD&D Insurance
    • Retirement Savings Plan

     

    Valassis Communications, Inc. and NCH Associates Paid Through ADP - To designate or change a beneficiary for your life insurance, complete the beneficiary designation form and submit it to your HR Representative. To designate or change a beneficiary for the Retirement Savings Plan, complete the beneficiary designation form and submit it to your HR Representative.

    Valassis Direct Mail and Sales & Marketing Associates Paid Through PeopleSoft - To designate or change a beneficiary, sign onto www.valassisbenefits.com and choose the "Enroll or Make Changes" option in the upper right hand corner. You are automatically the beneficiary for any life insurance you elect for your spouse and children. You may also complete a life insurance beneficiary designation form or one for the retirement plan and submit it to the Associate Resource Center.

  • Notify Valassis of emergency information. You should inform Valassis of any changes in your name, address or emergency notification information. Contact your Human Resources Representative (Valassis Communications, Inc. and NCH associates paid through ADP) or the Associate Resource Center (Valassis Direct Mail and Sales & Marketing associates paid through PeopleSoft).

  • Consider counseling through the Employee Assistance Program.

  • Consider revising or drafting a will.
