Change my status from Part-time to Full-time
|
Full-time benefits are activated as of the first of the month following
your change to full-time status (37.5 hours per week or more).
Select health care coverage. When eligible, you, your spouse
and/or dependents can receive the following coverage: Medical, dental,
vision. Please refer to the benefit cost
sheet for details on premiums. Valassis Communications, Inc. and
NCH associates paid through ADP should contact your local Human Resources
Manager for enrollment. Valassis Direct Mail and Sales & Marketing
associates paid through PeopleSoft can enroll
here by selecting on the link over to the right 'Enroll or Make Changes'.
Your user id is your social security number without dashes and your
default password is your 2 digit birth month and the last 2 digits of
your birth year (example: 0875).
If you are currently enrolled under the Medical, Dental, and Vision
plans your premiums will change effective the first of the month following
your status change, please review the benefit
cost sheet for premium changes. You can add or change plans but
cannot waive coverages.
Review Valassis-sponsored coverage. When eligible, you are automatically
enrolled for: Short-term Disability Insurance and Long-term Disability
Insurance (eligible the first of the month following the status change
- provided 30 days have passed since date of hire), Basic Life and Accidental
Death and Dismemberment (AD&D) Insurance, and Employee Assistance
Program (EAP). First of the month following the status change your Basic
Life Insurance and AD&D will increase to 1.5X salary versus the
fixed amount.
Determine additional insurance coverage needs. If you would
like coverage in addition to what is provided to you by Valassis, you
can select: Dependent Term Life Insurance for your spouse, domestic
partner and/or children and consider adjusting Basic Life and Voluntary
Life Insurance and Accidental Death and Dismemberment (AD&D) Insurance.
Evidence of insurability will be required for new voluntary and dependent
life insurance unless you are becoming eligible for the first time.
If you already have voluntary or dependent life insurance, you may increase
your coverage 1 level without evidence of insurability unless your coverage
is 3 times or $400,000.
Designate beneficiaries. You must name a beneficiary for your
Basic and Voluntary Life and AD&D and the Retirement Savings Plan.
(If you are married, your spouse is your beneficiary unless he/she consents
in writing to an alternate beneficiary.) Valassis Communications, Inc.
and NCH associates paid through ADP who need to designate or change
beneficiary information, please contact your local Human Resources Manager.
Valassis Direct Mail and Sales & Marketing associates paid through
PeopleSoft can designate or change a beneficiary by clicking here
and choose the "Enroll or Make Changes" option in the upper
right hand corner and log in to your information or by completing a
beneficiary form. Please click here
for the Retirement Savings Plan beneficiary form.
Determine Flexible Spending Account (FSA) needs. Through the
FSA, you can set aside before-tax dollars
to pay for eligible health and dependent care expenses that are not
covered by other plans. You can participate in a Health Care Flexible
Spending Account, and/or Dependent Care Flexible Spending Account. If
you set up a Dependent FSA, Valassis will match your contributions dollar
for dollar up to the first $600, for a total annual contribution of
$5,000 a year if you are single and file as head of household or if
you are married and file a joint return. If you are married and file
separately, the combination of your contributions and Valassis
matching contributions may not exceed $2,500 a year. You must be hired
by 10/1 to be eligible for the company contribution for the next plan
year.

to determine ongoing eligibility in the plans.
